YOUR REHAB PROGRAMME

After a severe acute incident, some patients may not yet be suitable to undergo an intensive, active rehabilitation programme. Rehabilitation provides a structured, goal-oriented treatment plan to help patients regain function within a certain time frame.

The transitional phase for some patients may be prolonged due to the severity of their condition and tolerance level. They may have just been discharged from extended acute care, such as ICU or HDU setting, and may experience a low level of physical or cognitive functioning.

A steady start. Slow Stream Inpatient Rehab provides the opportunity for patients to get a gradual start in rehabilitation through an adjusted intensity programme tailored to their specific needs. This flexibility accommodates the patient’s current needs and strives towards achieving their rehab goals.

Recovery will be measured and scored at the start of the treatment plan in terms of level of independence by the Rehab Physician and rehab team. The score is reviewed weekly until discharge to ensure the treatment plan is on track. In some cases and as recovery improves, the Rehab Physician may suggest moving on to an Active Inpatient Rehab programme if suitable for the patient.

YOUR INPATIENT JOURNEY

Our Case Coordinators are qualified nurses who represent ReGen Rehab when they meet you and your family for a clinical assessment and family counselling. This assessment will help you discover if rehabilitation is suitable for you at that time and should be done as soon as medical conditions are stable during your acute stay or at your home.

When you choose to begin rehabilitation with us, our Case Coordinators will help you plan, arrange and organize your transfer and admission to ReGen Rehab. Once you are settled in, a detailed multi-disciplinary assessment will be carried out by the rehabilitation physician, therapists, and rehab nurses to develop a custom programme to address your specific condition and recovery needs.

Your daily individualised schedule will comprise direct and indirect therapy. Direct therapy takes place at the rehab gyms with your therapists for up to three sessions a day. Indirect therapy reinforces what you’ve learnt during your time in the ward through activities of daily living (ADL) and is supported by the rehab nurses.

Our rehabilitation programmes are desiged with the common theme of assessing individual needs of patients and developing a specific goal-oriented outcomes as part of a comprehensive treatment plan.

In the rehabilitation treatment plan, patients may undergo two streams of therapy depending on the underlying medical condition and endurance:

Active stream: Up to three hours of direct therapy a day, supplemented by indirect therapy a day by the rehab nursing team with activities of daily living (ADL) therapy.

Slow stream: One to two hours of indirect therapy a day, supplemented by indirect therapy a day by the rehab nursing team with activities of daily living (ADL) therapy.

Each patient is cared for by a multi-disciplinary team of medical practitioners, led by a rehabilitation physician, comprising: Rehabilitation nurses, physiotherapists, occupational therapists, speech therapists, pharmacist, dietitian, neuropsychologist, and others.

Periodic team conferences are held for continuous patient management and monitoring. Weekly rehabilitation goals are set according to the treatment plan and reviewed. Family conferences are an avenue for patients and family members to discuss with the rehab team as a whole and can be arranged at any time.

For patients coming from acute hospitals, our Case Coordinator will perform a rehabilitation assessment at the current residing hospital for suitability. Upon discharge, we will assist with arranging the transfer to ReGen Rehab Hospital if ambulance service is required. Upon arrival, admission and registration will be done at bedside and a three-point assessment will be conducted by the rehabilitation physician, senior nurse and a therapist.

We advise patients to stay for an average of two weeks in the first instance to allow sufficient time for patients themselves and their families to understand and journey through the rehabilitation programme. Discharge planning begins at the time of admission, encompassing the goal-setting process with the Rehabilitation Physician, goal-review during the team and family conferences, and care giver training towards the planned discharge date if required.
